Helpful hints to use the Walmart Rx Card:

• 1. Click on the link below for your one page Special Olympics MA® - Walmart® form. Bring the form to your Walmart®, Sam's Club and/or Walmart® Neighboorhood Market store.


• 2. Feel free to print off additional forms for family and friends or your wallet sized card. Please note; form needs to be presented upon initial visits to pharmacy.


• 3. Present the form to the pharmacists; they will then load you into the Walmart system using the identifying numbers located on the bottom this form.


• 4. If you currently have other insurance, please present both cards, pharmacist will provide pricing for both. As the consumer, the Special Olympics MA® Walmart program brings you choices, please select which program fits your needs. IMPORTANT!!! The Walmart card cannot be used simultaneously with any other insurance, referred to as "double dipping". Anyone who currently has a deductible associated with prescriptions please note that card can be used but discounted Walmart price will not be applied towards the deductible.
